Displays a form within page content.

Key features

• Required title for the form
• The form will display in a bordered block. The optional intro and content after fields allow copy to be added before and after the form within this block.

Important when adding a Gecko form

• When creating your form within Gecko, please ensure the 'Custom CSS' area is filled in and contains the following: @import url("https://www.derby.ac.uk/media/global/dev/gecko.css");
• When creating your form within Gecko, please ensure we are only collecting relevant information that is needed
• Forms must contain privacy statement to inform the user how their data will be used
